Make sure to include any video evidence.
And for what it's worth, I already recorded this guy hacking yesterday or 2 days ago, but it can take a week or two for the moderation team to actually process the report... so even if you don't send a video, this player should get taken care of soon enough for the hacking part.
For the doxing part, I'm not sure what happens when someone gets reported for 2 different offenses at the same time... are the bans cumulative? No idea. I suppose it is still worth a try for you to send a report about the doxing part.

Not sure of the exact process as I've never had to contact them to recover an account, but you can select "Hacked account or login issues" for your ticket
If you appear to get an automated reply, make sure to reply to that email. It will make it so an actual human will look at your ticket.
