The room does literally give you a wings of wrath, and it's the only usable item in that room.

The same one that allows you to use any other item in your inventory bar. The right stick in your case, if your screenshot is yours.
Did you... play all of heretic with using any inventory?
It does literally show you the button to press in the hot bar.
Heck, weren't you at all curious as to what that even was?
You do need the wings of wrath actually selected in order to use it.
Is it though? Was this the first time you played heretic?
It seems more like you had a group of people who through an unfortunate combination didn't realise the wings of wrath was there, and didn't know what the wings of wrath even was.
