Not sure if it's PAPI or Luck perms, but it seems like the placeholders are not staying when a player leaves the server.
Of course, it is completely verified that the config for DiscordSRV is correct since it displays correctly for everything else, and the DiscordSRV team believes it's PAPI that is the issue.
(Ignore the no prefix for the died message, that was my fault. It is only when leaving that definitely doesn't return prefix)

unsure which placeholder you're using, but it could be that at the time DiscordSRV parses the prefix placeholder, the player was already unloaded and the prefix was therefore not in memory
