You are only blocking 55% of all melee hits against you due to a low uptime on Shield Block and possibly having mobs attack you from various angles rather than from the front
It also looks like you didn't cast avatar a single time throughout the dungeon? and only activated it through your shield wall usage which could also be improved upon
