Situations like this where you execute in very fast succession indicates that you likely do not pay a lot of attention to maximizing and extending your juggernaut uptime, causing you to lose the buff throughout the fight. Your opener on Sprocketmonger was kinda weird (you mortal striked between cs and bladestorm) but it's all in all not too bad and you're also utilizing your overpower quite well, not wasting martial prowess. Idk you seem to be doing a solid job, hence your 95 on Sprocket. My best advice is to emulate that single target behavior onto other bosses before you move on to optimizing aoe situations.
To optimize aoe situations (e.g. Gallywix adds) it's important to think about when to use cooldowns in order to line them up with the next set up adds. A lot of players prefer not doing that themselves so you could look these timings up by checking rank 1 parses.
On bosses such as cauldron of carnage you might want to look into pre-avataring 10 seconds before pull in order to have it up for the intermission 👍
FWIW the ST guide does specify that you cast MS after CS and before BS
