My verdict is that if there is no safe guarded minimum of procs, with some tests and beforehand anecdotal info and personal experience (4 TD procs in 8 seconds -Morrison and my 3 TD procs in 7-8 seconds)
it is indeed completely random
getting multiplicatively (could be wrong on this) less and less likely to have a proc in the same minute, but with the ultimate amount of luck factor

How do you know/decide when to use shield wall or last stand when doing trash? I tanked a 10 and a 9 last night and I was really struggling to use them. I feel like sometimes I’d use it and it was unnecessary but sometimes I’d use it and it’d feel like it was way too late. Is it just practice/getting to know the dungeons or??

The question of Last Stand comes down to your Shield Block downtime. You don't have SB charges left? Use it
Shield Wall is nowhere tricky either. You either fill in open spaces between SB/LS or use it to pull really-really big or proactively to survive big damage spikes

As long as you are highest on threat they wont melee someone else. There are some mobs you shouldnt kite and where you have to be in melee at some time: like those Shield-Guys in atal. Their Shield Bash will hit someone else in melee range if you are kiting. But in general CnS is big for kiting
