@ruby nexus cyclo trinket works based off of ilvl yes, it does good dmg even with lower level cards, but given that they are only dropping from m0 or lower they cap at max ilvl of 455 And you can only get a yellow of said ilvl. The red card is always 415 thus the trinket is out of date

To clarify, what you're likely seeing is people going into a previous portal to meet up with the tank at the next obelisk. This helps in some situations to manage the adds (e.g. you have the entire group in the phase out of LoS of the blob lieutenant, tank spawns him, all tentacles port to the group and you get to kill them without slime all over the floor/group)
Each lieutenant can only be spawned at their obelisk. If the obelisk for that lieutenant is not activated by at least one person, the lieutenant won't spawn. But like Speg said, once the lieutenant is spawned, he's in the realm no matter which portal you enter by.

Basically, plan out your CDs (at least have a general idea) and roll them accordingly.
LS should be used asap because of Bolster, Demo Shout and Avatar are basically used on CD, more or less, and SW should never be saved either.
Use Commanding Shout on tough packs as well.

If in M+, I would use Heed My Call. A lot of folks talk about it breaking CC and stuff but I've always used it on my toons and never had it break any CC or pull anything it shouldn't.

So first of all, you don't fight much that stuns. You either skip to the left or you can skip on the right if you have the Gnasher food
Secondly, you need to get the WA for Gift of the Titans that shows you the internal cooldown of it
and you make sure you start Rexxar when the gift ICD is up. As soon as the first add shows up, you kill it and it should proc Gift. Then you just yolo the rest of the fight since you're immune to sanity damage

I taunted last night on a tank swap, and he never changed target(taunting a 2nd time brought him over to me).
