Void emissary on right side, 2nd to last pack - the pack right before the single stealth mob - no easy place to LOS void emissary. We just went all in on emissary to kill it before second cast.
Void emissary in the pack before 2nd boss too. Had evade issues pulling the pack down out of LoS. They’d reset and run back to the top. It was great. Decided to single target emissary there and then deal with the pack
All other emissary placements that we saw seemed pretty inconsequential

If everyone stacks on tank Last boss UR and then runs through as he begins his frontal cone attack spray, but it seems like he also drops grey around his feet randomly. Some ppl in my group thought it was targeted and if we don't run at same time that was reason it was landing on party. but after several experiments with that. We concluded that he does frontal cone, and then random circles at his feet. so we just faced him towards grey floor then got 5or 8 yards away from him to avoid ground dmg.

Weirdly enough, we actually continued the dungeon later in the week after attempting the 4th boss for a while. When we came back, the 1st boss (King what's-his-name) was up again with HK-8 overhead, so we had to do that boss on hard mode, but when we got to HK-8, he wasn't hard mode anymore.

Yeah you should use it very frequently; with the Anger Management and Bolster talents, you should be getting about a 1 minute 15 second cooldown on Last Stand if you're doing your rotation properly and not overcapping rage.
And you should prioritize Shield Block over Ignore Pain; a pretty common newbie prot mistake is they think Ignore Pain is the main mitigation and Shield Block is just backup. In reality, you should use Shield Block as your top priority and use Ignore Pain whenever needed.
