If you have an item that has more secondaries (such as a socket, or a trinket with secondary stat proc vs. damage proc, or a regular weapon vs. the Waycrest Manor cleaver), it might be better.
What "ilvl > secondaries" means is that we don't care what secondaries items have because they all have very similar value.
So our choice is simple - take the item with higher item level because it has more stats.
