the little I can manage to guess is that the "second big bang," as spoken of in the other trailer(s) split the human populace across different dimensions/realities/whatever and the ways to connect to the other sides is to use fetuses. in-between the neighboring dimensions are weird creatures that are not humans at all but weird alien looking figures
also the mads character might have a backstory that lends itself into what happened to even cause the initial division of humanity, but that's one of the more "out-there" guestimations. you can see him before he looks super evil in way that makes me think more "mad scientist" evil

also it seems like when you kill something you're punished in the form of having to lug it around, but that's another one of those "guestimations." remember - Sam's on the "Corpse Disposal Team," which means he has to dispose of corpses.
