Something went wrong errors

I keep getting this error repeatedly. “Something went wrong while processing your request. Please try again, or use /new to start a fresh session.” even after starting new session I get this error

That message is the generic “the agent run failed” wrapper — /new only rules out a bad conversation state. If it happens immediately in fresh sessions too, it’s usually gateway/provider/auth/model/tooling failing underneath.

First thing: what version are you on?
