Can you share your logs.zip file found in %UserProfile%\Zomboid?
And can you share a recording of the issue?
This looks like something outside of the game is messing with it, do you have anytrhing running in the background?
i have medal running in the background but i doubt it would cause stuttering because this game isnt very difficult to run. btw i checked task manager and none of my stuff is at 100% usage

I had FPS issues with a GTX 1080 and an i7 9700K, bought a 5060 Ti 16 GB yesterday and my game now runs smooth as butter lmao, B42 is literally unstable in every way you can think of, bad optimized, 50 bugs appear after a bug fix, it's crazy, if you want a decent gameplay play B41 until they decide to address optimization/bugs issues or be an idiot like me and keep looking for fixes every time they fuck up something
(btw before latest unstable updates my 1080 could run this game smooth as the 5060 Ti is doing now)

Game still runs perfectly smooth on my 1080ti, 5700xt, and my 4090ti...

Update all your drivers. gpu, cpu, mb, everything...
Disable any unnecessary startup programs. Anything that uses your cpu.
This game heavily relies on a single thread so even though it says 20% cpu usage in the task manager, the game is probably using nearly 100% of one of your cores/threads and any little background programs are fighting for those resources.
There are ways to force the game to use specific cores/threads but atm you have to do it every time you start the game unless you write a launch script.
Or use a mod...
Hopefully in the future we can get some native multicore support cause my 16 threads could use a workout lol 
You might not realize it but GTX 1080 vs GTX 1080 Ti is like comparing a 3080 10 GB LHR to a 3090
In this game there shouldn't be a noticeable difference between the two. You said yourself that it was running smooth.
Then instead of trying to fix your issue you went and bought one of the most expensive gpus on the market to compensate lol
And I really hope you think it helped cause otherwise you just wasted $700+
But your issue imo might have been your cpu actually. So next time you see the game stutter, because it will on any gpu, just go buy the most expensive, fastest single core cpu you can find. See if that fixes it lol 💸

I bought a 5060 Ti and it solved it, before the latest updates my old 1080 could run this 2D game smooth as the 5060 does now, it's crazy, Idk what they are adding/doing to the code
