I thought it was the problem with mods and tried testing the game on vanilla and it's the same, reinstalled the game already, cleaned up docs and saves, set all of my settings to lowest possible. Is the issue with this my pc? Population settings on the vid are 2,5 population, 1 starting and 0 multiplier. I have already tried allocating ram but didn't do anything.

The game works perfectly fine when I'm standing in place or when I'm in rural area, or anything beside cities. Running around to let the terrain load etc. also doesn't help at all.
My pc specs are:
2080 rtx super
16 gb ddr4 ram
7 3800x 8 cores amd
I also do have the game on ssd.
Dedicated server or just single player?
im only playing single player
hmm, 16gb of RAM is a little low but... shouldnt be holding you up that badly.
on a server with a similar build we had the same issue, it was basically IMPOSSIBLE for us to walk different directions, especially if cell loading was happening. If we were in our house it was fine.
We updated to 42.14.1 and it sort of just solve that for us... i cant really explain it but it runs radically better.
The best thing I was able to do for my own performance was turn of shadows for as much as possible.
you could also try connecting to an internet server, just to see how it runs.. perhaps your machine will run better if it doesnt have to do ALL the thinking...

from what i have figured out it was the cpu temperatures
that'll do it for sure
it did not fix it entirely but still
now it drops like to 50-40
which is playable

yeah, about only other thing I can think of would be to try connecting to an internet server that is maybe empty and near you (ideally) and see if it runs any better...
ive figured out its dropping due to amount of zombies

and the reason it was dropping so low was cpu
on 0.65 population its stable whole time
and yes, you are likely getting about the best you will with that current set up..
im just happy i finally get to play the game
perhaps you can simplify them if you add more,
make them leash shorter, wander shorter, basically limit their ai processes
