I would recommend they uninstall all mods and verify their files and only install mods you need for whatever server your playing on and no more than that.
As an initial troubleshooting step. I dont actually know of that issue or how to fix it but usually when its localized to one person is a file mismatch probably caused by a mod.

If your friend is still subscribed to mods, unsub from all of them and verify or reinstall. Just disabling the mods may not be enough.
If they are vanilla build 41. Not sure where something like that setting would be. But if its not a mod, and its only him, my next guess would be a setting or hotkey got toggled. Keep in mind I dont know if thats actually the case.
If its none of those a fresh install is worth the try.
Beyond that... no flipping clue. I haven't played on b41 in years

That ping is extremely high, is your friend able to reproduce it on servers with lower ping?
I recommend playing on servers below 100 ping
Anything above that will start having issues of varying degrees

High ping means the internet is bad
Ping is the time it takes for data to travel between client and host
Meaning the only thing that affects that is the distance between them and the internet speed
And any issues related to the internet

Since that would switch the issue from it being network related to being a system issue,

Seems like it's client side from your friends not seeing anything. If the server was an issue I'd assume they'd be having similar problems.
Have you increased your allocated RAM for Zomboid? If you're running with a fairly decent amount of mods it could help to bump it up if ya haven't already since how much RAM the server has is separate from how much RAM your game has.
You can do so by:
Go to the game files, Steam\steamapps\common\ProjectZomboid
Open ProjectZomboid64.json with notepad or any text editor.
About half-way down, under "vmArgs" there'll be a line that says "-Xmx3072m" Increase the 3072 to a higher number like, 4096, 5120, 6144, etc. Always multiples of 1024.
I have mine set to 8192 while running a modpack with around 480 mods and it runs smoothly.
