I'm running a server with very slow time (2 game days per real-life day), and players are starving but are still gaining weight. I tested this by taking 'hearty appetite' and going 48 hours and only ate lettuce when starving and exercising to exhaustion. In those 48 hours, I gained 2kg. What am I missing?

Did you check your calorie intake and expenditure?
You can do that with several mods or debug mode for example

You would have to launch it with the debug flag and log in as admin
It is possible that the 2 game day timer might mess with the calorie calculation
Which is why I am curious as to what the calorie intake looks like

That was my thinking as well
It turned out to be simpler though; Several of my players were on a strict diet of MRE's from the MREM mod, and those have unreasonable calorie counts. I had expected them to be ~1k calories, but they were coming out to double that.
I did a bit more testing with other foods, and it looks like calories are being calculated correctly, even with our very-slow timeframe.
