Undiscoverable Altar

So, Ive discovered every location on the map except 1. I am 26/27 in the Swamps. After hours of checking every gray sliver on the map, I broke down and used the Windrose interactive map see what I was missing.

Turns out, the missing location is an Altar that Ive aleady interacted with. I dont remember which one it is, but it says "Better get out of here before you become someone's prey."

Its right next to 4 other POIs that I have discovered. The map around it was colored in, just no icon. I have 100% been here before.
Now that I know, I cannot get the icon to show up on the map. The interactive map says the location has 2 chests. But when I go there in game, there are no chests and it doesnt show 0/2 like it normally would.

Is there any way around this? Is this save file stuck at 99.999% completion with 1 location permanately undiscoverable? 🙁

I did not. It was literally the last thing to do in that playthrough so I didnt spend much time on it.
Was hoping for an answer here 😔
