When u unequip a backpack and place it in a backpack slot that will dissappear after the actual unequip takes place the backpack is lost forever.

Its on the ground, sometimes it flys far, I reported this yesterday then found it on the ground where i took it off 🤙

As Jono said, when this happens you throw the backpack somewhere, and our pirates have been working out because sometimes they can throw it quite far.
