I have picked up multiple items in different zones and it is not updating the discoveries, ive restarted my game, dropped and picked them up again, etc.
I am playing modded, i can provide list of mods if required. but played an entire world with the same mods and am just now starting to find this issue.

Thanks for the report. Unfortunately, we can't troubleshoot modded issues. It is known that mods cause discovery issues (and many many other). And those issues don't happen immediately after installation in a lot of cases.
All we can recommend for now - try uninstalling all mods (and mod manager apps), reinstalling the game and restoring from backup.

Okay I just found a possible fix for this. It is a program a developer (not one associated with the Windrose team) on Nexus that made a program that scans your save for the thing corrupting the discoveries/progression and fixes it for you. I just tried it myself and it worked perfectly. Its called WSE Project - Save Fixer. I am not saying it will 100% work for you but this was the first thing that actually fixed my problem.
