Was your inventory full at the time? If so, you may have dropped it at the quest completion.

Not really a bug. There also is not a way to get another one once you finish the quest. It likely drops on the ground at the alter when you do it if you are full inventory. They probably need a method for quest item rewards when inventories are full (like mailbox, or a chest delivery) or allow for it to be repeated with no xp reward.

I have the same issue. I completed the quest and got nothing. I think my inventory was full, normally is...I have been back to look for it but nothing on the ground.

Items despawn I think after like 6 hours? not sure the exact time @amber jacinth Currently there is no way to get another whistle, that I hope they fix/address.

There are chances it's laying / floating somewhere around.
For solo/offline items sees to not be despawning at all (i have rocks laying inside mines for days already).
When something is dropped on the ground (full inventory) - its not directly under you. Could be in 5-10m radius, look around in a wider area (and above too).

I went back...nothing. I built up to see if it was on top of the statue. cleared out all the grass and bushes. Welp, no whistle for me
