I've only seen this once, but it happened when I tabbed out of the game to read a text or something while it was loading, which broke the UI pretty bad, but I played a game anyway with the broken UI and then 456 was there on the main menu.

also the ghost 456 as he appears on the home screen was also there in a game of RLGL with the broken ui state

Okay, this error happened to me finally. It was an error string that started with "Minigame", then loaded to that.
