Improper opener
Should press Shiv much more often
You let your garrote drop a few times
Don't ever press Ambush, feel free to remove it from your bars. Only use it if you play Blindside and get a free proc.
You wasted some combo points with Mutilate, only Mutilate to max-1, no more than that

Definitely fix your opener asap, it's the easiest thing

And not necessarily, it says that it increases in effectiveness with more powerful shards.
