Well, for one, you're playing with The First Dance, but after casting Shadow Dance (and gaining 4 CP), you cast Gloomblade -> Shadowstrike -> Gloomblade without using a finisher, meaning you overcapped combo points by at least, uhh...4 (since you also get Premed from the next Shadowstrike after casting Shadow Dance), ignoring additional potential CP gained from Shadow Techniques, Seal Fate etc, and after that, you used Cold Blood on a Gloomblade. I dunno if you have Cold Blood on a separate keybind, but if you do, you can just macro it into Secret Technique, 'cause you will always spend Cold Blood on SecTech.
Also, a lot of times you use Shadowstrike right after Shadow Dance, which will overcap you on combo points, since you get 4 CP from First Dance, 3 from Shadowstrike + 2 from Premed (adding up to 9), and in this bit, for example, you use Gloomblade after your first Eviscerate in a Shadow Dance, and then spend Secret Technique on a single Gloomblade as the only builder ability, which probably was far from being high CP.
Also, save your second Vanish charge for after the next time you use Secret Technique with Cold Blood.
And you did get Find Weakness on that pull, but it's always good to Shuriken Storm after the first Shadowstrike to guarantee a Find Weakness proc (and get your CP up for a longer Rupture in the opener).
I suggest looking at the opener in #subtlety-faq (the "with The First Dance" one) and just practising that until you've got it down, 'cause all your other Shadow Dance goes will be very similar anyway (ignoring everything before the actual Shadow Dance cast in the opener, of course).
