Off the top of my head, packs that have < 3 mobs in KR include everything but like 2 packs leading up to the first boss, half of the gauntlet room, the bridge after the gauntlet room, possibly golems in the second boss room depending on your tank. A few packs between 2nd and third boss are only 2 mobs if your tank doesn’t pull extra, also Zul.

If you have OMNICC you might have to blacklist WA though that sounds odd.
Try going into the Rupture aura that you are having an issue with, going over to Display, and unchecking the checkbox next to the 2.
Usually this is an issue with cooldowns and not debuff tracking (ie. Blizzard and w/e other addons have their own CD tracking and it overlaps).

@somber dagger don't vanish garrote until your original garrote falls off (assuming opener). mid fight you wanna vanish double garrote during vendetta to maximize the SS traits bonus. i generally like to do this during daggers poison bonus for the auto full cp envenom right after

i'm not surpised. it makes your opener on bosses more consistent, as with vanish + vendetta you get a really long high dps opener. you have stealth between each trash to abuse the trait. and with subterfuge + SS, a 3-4 garrote opener is amazing. it's full cp + 3 h yper buffed garrotes + solid energy regen . . . . it's nuts
and they fixed the bug causing ss dmg to drop off on the 4th garrote
i usually 3-4 garrote > envenom > fan of knives
while I could rupture with the cp. the envemon > fan makes sure that poison is up on all targets, letting the energy regen from 4 garrotes start coming in. in case some of them didn't get poisoned
