Doing a Death Touched Acolyte/Cult Initiate run in Tomb of Ulth Vuul and got to the point in the screenshot where only Teleport was still empty but rune pickups would only credit me a reroll rather than open the menu to slot for Teleport. Picked up a few before I took this screenshot to make sure it wasn't a one-off, but took the rest of the run for a good 30+ minutes and it never resolved.
Haven't tried to reproduce it as that one run was enough for my gaming for the evening.
