I've got a mega bad case of Vertigo right now, but you can just copy my mod and change the relevant things!

but yeah manually download this, change the version number in the manifest to 1.0.0 and edit the description in there aswell, replace the png icon and replace the text in the README and then add your own MP4 files instead of the ones in there already
