#Xbox controller keeps randomly turning off

I’m playing my Xbox and every 5 minutes to maybe even 5 seconds my Xbox controller just turns off. I have replaced the batteries and the controller is relatively new. Does anyone know a solution

Also for some reason the battery icon when you click the home button is showing a bar with a power icon instead of the bar being filled showing the percentage
