Nat Type: Moderate, UPnP not successful

I have tried everything to make my Nat Type open and my UPnP to work but its not helping. Sometimes when i restart the console after changing the name of it or clearing the Mac address the Nat Type will be open and then goes back to moderate shortly after. Ive called my wifi provider and they told me everything is working, this problem randomly happened with out any real reason. PLEASE HELP
