Recently I attempted to log into my xbox account and it told me that my outlook account had unusual sign in activity. When I tried to verify the number it didnt send a code. I am still getting notifications from the xbox app on my phone of my friends going online but when I open it, it gives me the same verify page. I can open my outlook email account but I'm not getting any new emails.

You should change all the passwords on sites where you’ve used the same password.
You should also change the passwords on any site/service that you “lent” your password to someone else to use.

It is almost never a “device wide hack” it is almost always reused or shared passwords.
