My Xbox makes a terrible fan noise when launching/playing games, and never stops. It even does it on the home menu. Can I fix it w/o opening it? Thank you.

if it is dirty, you can wipe it with a soft barely-damp cloth on the outside and that will clean it.
if your fan is making noise then you should contact support for assistance. a little bit of dust is not making your fan sound terrible

regardless, if you think your fan is busted you need to contact support.
there are no user servicable parts in your console so there is nothing you can do to 'fix it'
