I've had this occur more in Engineering (Standard), but if you flee from a fight and dungeon recall back, there's a chance that you will be teleported to a separate instance of the current sigil. I wanna say that this is because Doom Moon's player limit is very low, so if it's full it swaps you to a different realm or area, but I don't know the technical aspects that well.

Did you have a team with you in the encounter?

Yes, it was a full team. We were grouped with sigil preference if that has anything to do with it

So an adventure party?

would you happen to remember if you could walk forward and go to the second? Could possibly the bonus mobs respawned in the first room?

I could still proceed to the 2nd and 3rd rooms, but everything respawned in the split instance. All 4 wizards in our group were at Montgomery, but in groups of 2 (we had 2 people flee beforehand and both of them got put in the same separate instance)

It just happened again, here's what else I can tell you

4 of us are in an adventure party group. We just finished a run, so one of us went back outside so the rest could teleport to them to do another run. 3 of us made it outside in greyrose realm, but there was a 4th random person there which disabled teleports because I was suddenly "busy", so we switched to Satyr realm.
We did the instance again, 2 of us had to flee the first fight because we had the wrong decks equipped, and those two are now in a separate instance in Greyrose realm

All 4 of us have remember realm on in settings, I'm not sure if that has anything to do with it though

Okay I THINK I can consistently reproduce it now

I just tested in an adventure party and solo, it didn't repro solo but it repro'd in the party. Here's the exact order I did things. IF YOU:

Are in an adventure party
Start an instance of Engineering (Standard) and don't complete every single fight (for repro I did the 1st fight only)
Leave
Switch realms
Start another instance of Engineering
Flee and dungeon recall back

Upon teleporting back after dungeon recalling you'll be put in the realm you switched from and thus a different instance.
