The reward would be easier to get if you're late into the run with great econ, but then the reward would be less needed, so it wouldn't be super broken if you get it late-game. The number of items might need to get tweaked, 50 was just my first thought where it didn't seem easy but not inachievable.

I'd say it isn't retroactive and you do lose it, if you didn't lose the extra slot you could get yet another one by finding the joker again
