Does the card that gets selected work like how idol selects a card?
Or like how cerulean bell selects a card
Or some other way?

Does it choose a card in your deck and reset if you don't play it that round or does it choose a card in your hand and reset if you don't play it then?

having to fix towards the second hardest hand type in the game to draw isn't a good thing
Straight flush is harder to fix towards
It could also be an uncommon scaling flat mult or chips. Not sure what's best.

if it works like cerulean bell it's basically a better madness
0.5x is probably overtuned; you can always just waste a hand on high card

.25 sounds fine
it's 3/4ths of the rate that madness scales and doesn't have a too impactful gimmick

Thats still alot of Xmult for HC and sorta too demanding for smth like straights.. tho w deckfixing i suppose it could be fine

Fair enough, there are enough jokers that enable HC and Pair strats.
there are also too many mechanics that limit 5 card hands

yes
it is a lot harder to fix your deck to a good flush house ratio than it is to just copy all your cards into the same rank

through death/dna/strength/suit changers/etc
stength and death also assist for deckfixing to full house
and suit changers help either of them equally
its a lot easier to fix towards one rank than a 3:2 ratio of two ranks
i'm talking about the difficulty of deckfixing to draw the hands atleast once per round
there's also not too much incentive to deckfix for either when 4oak is sufficient

If you make it this way I’d suggest making it select like idol

i could see it being fun as an uncommon and only caring about rank
All you need is a moon, sun, world or star. And then draw a full house with 2 of that suit
It's still more difficult then 5oak but not by much
