I've got a hand size of 5 and just used immolate in a mega spectral pack, and now I can't do anything.

No, since it isn't actually possible to save after having used immolate (or similar), and reloading the game just puts you in the spot before having used any of the consumables

Yeah just reload the run and it'll be fine
