It would be cool to add a little bit of noises in the background or have the sense of unease like something’s behind you when there’s just a streetlight or a mannequin. No jumpscares or entities just that feeling of “I always feel like, somebody’s watching me.” (See what I did there)

The problem is, if it is implemented, we will always know that it's fake and there is nothing there. The tension only works if the presence of an entity is unknown.
I would rather have an X percent chance of there being an entity, and you don't know if the level you entered has one or not. And then just have the sounds and mirages constantly. Then everything you see and hear could be a real thing... or not.
