This happened last night. I do know it was the new main Well with the questline, and it was not far from the save point at the top of the Well. I'll have to make a trip back there and get a screen shot.

So there is a ladder there, but it can get destroyed by enemies and or by players, like Im pretty sure I destroyed it with my fireball.

Yes, went back there, there was a wood ladder, but the ladder got destroyed, so couldn't get out.

I think a grapple point or something that can't be destroyed would be better. In this pic, the ladder was already destroyed by the mob, and it easily will as all the enemies start attacking you as soon as you start across this section.
