My monitor is 144hz, and in-game fps is 40, so the expected fps should be 120, but the fps counter displayed 144/144, which is not true. This problem can be replicated by closing minecraft and reopen it. Reboot is the temporarily fix for this problem.
OS: Windows 11 pro 24H2
Minecraft version: Java 1.20.1 Fabric
My spec:
14900k
RTX 4090
64GB RAM
Driver: 555.85

Update: This happens to not only Minecraft.

Had the exakt same thing
Tested Hogwarts legacy at a 48 FPS cap
Sometimes it showed 48/144 and it was really smooth
Other times it showed 144/144 and was really jittery

Happens with me too in Company of heroes 3, rebooting pc is the temporary fix. Sometimes frames get 4 times the capped fps. I cap it to 37 but it shows around 130 fps. This issue started after the last official update. Earlier was using the beta versions.

it started to happen to me too in all my games too since the update, I was using driver 552.12 and did a fresh install of 555.85 to see if it would fix but nothing edit: laptop dumb, it was messing with LS

this has been happening to me too on AMD, i only got it to fix once by switching to admin mode, but after reboot it never worked again
I've tried it on SQUAD, Lost Judgement, Rust, Overwatch, and VLC. The fps is misread on all of them.

Also the issue is random across both GPU platforms, i see other people are having issues on nvidia cards but I have a laptop with a Nvidia gpu and frame generation works perfectly fine on that one

So after a clean install and testing few days, this problem become inconsistent. Sometimes I just rescale, but sometimes this way won’t work.

Also, rerolling driver, admin mode, downgrading LS aren’t working either.

What works for me:
Either click on the desktop shortcut in the bottom right corner of the task bar and then click the game's icon again (however on multiple screens that will cause discord on my second screen to disappear too and when I click on the discord icon to get it back it sometimes will read my desktop's FPS again)
Or while ingame press CTRL+Shift+Escape to open task manager while the game is running in the background and then close the task manager again (a bit more inconsistent solution, the above one works consistently for me)

I think the reason is that something inside of windows sometimes takes over LS's focus.
The first method might work consistently because when clicking on that icon every program is automatically minimized and when you click on the game once more only that one will be in focus

A user messaged me earlier with a fix: Set your second monitor as your first and vice versa. Then try it again on your main monitor (that is now selected as your second monitor) and it shouldn't happen anymore

What I initially did to fix this problem:
Play the game on the second monitor
And set LS to output to my first
Then it was pretty much like with your game currently - game was on both monitor's but the smoother gameplay was on the main monitor

However what you're supposed to do is set your second monitor as your main in windows and your main monitor as your second

My second monitor (the Samsung) is now set to my main monitor.
Using LS I made the game show on my main monitor (asus).
I’m getting 120 fps, not quite the 144 but still great. But now I’m getting the game on both my monitors

Okay so it probably took the information of the samsung one when you played it on that screen
Try restarting the game while it's on your asus monitor and then chances are that it A) should launch on the Asus one and B) should know that it can use 1440p too

Another method that would be worth trying is using AMD Super Resolution to set your Samsung Monitor to 1440p

i guess you need to remove the windows activate water mark because it acts as an overlay and losless scaling will focus on it instead of your game and it will not work good if you already activated it then try to get rid of any overlay you have everything becuase its detection issue

So using @untold path's method actually worked, though I'm facing another issue. The first time I use scale, everything works perfectly. But when I alt-tab and go back to the game, Lossless Scaling crashed and I have to restart it. The issue seems to be game-specific, as it happens in Batman Arkham Asylum, but not in Kingdom Come Deliverance.

Earlier I discovered that my own personal fix (clicking the desktop shortcut and then on the game) didn't work because it minimized every app to help LS focus on the game alone but it was due to Discord then being inactive on my second monitor (usually discord is always running there)
When I minimized discord on my second monitor it immediately detected the FPS correctly.
Then I told the dev and he send me the fix regarding Discord hardware acceleration. Upon disabling that the FPS have been detected correctly even with discord being active on my second screen

Also, with the drivers unistall, my monitor went back to 60 hz but when I put it back to 75 hz, the framegen works perfectly.

Update: I reinstalled all the drivers and now I have updated drivers. The program only works if I open the game with 60hz, my monitor is 75hz so all I do is boot the game while I have my monitors refresh rate to 60hz, activate the program to check the game if the fps are being capped correctly, I have mine capped to 35 fps. If the fps shown by Lossless Scaling are correct (you need "Draw FPS" to be active of course) now switch to 75hz when game is booted but not being scaled. Now I'm at 75hz and when I boot Lossless Scaling, framegen work perfectly
