I reckon they should add a feature where when you have opiate withdrawl the visual effects get cranked up to max rendering the game practically unplayable until the withdrawl wears off or until you take more opium and maybe the visual effects get worse the more addicted to opium you are so opium addictions and withdrawls are more of a feture than something that gives minor effects that you can just ignore. Maybe some players may find this annoying so perhaps it should be a toggleable feature that you can turn on and off in the lobby creation. By health adjustments I mean adding individual body part health and major consequenses (not just death) to having these injuries like maybe if your leg is slightly damaged you would have a bit of a limp or if one of your arms is slightly damaged it would be harder to use weapons or if both of your legs were severely damaged you would crawl along the ground (like the happy wheels damage system) I know the health one sounds ambiguous but I would love to see it in the game if its too much I hope I get to see the opium addiction one

i think addiction being more is fun as long as its not overdone
like making it have up and downsides
like with opium you vomit everywhere on withdraw but as long as keep taking it you get a passive healing
chem addiciction could do something similar as downside but give you a minor speed buff
alcohol could have and addiction too but psychosis resistance is not that important personally
i dont think it really should have upsides, as it is meant as punishment for having too low medical skill, so having it on purpose shouldnt be a thing you can desire
