As for frequency of this happening, I got this today, but today also was the first time someone in my team used "Random Gatekeeper" option.
I think it has a chance to happen every time someone uses the "Random Gatekeeper" option in co-op.

On related topic, the "Random Gatekeepr" does not equip the Omni suit. You start as a random Gatekeeper with default suit.
Proof, again, on the screenshot above.
I picked Mediator with Omni suit.
The other player used "Random Gatekeeper", and got a Mediator with a default red suit, even though they own Omni suit, too.
This happens every time on every Gatekeeper. I also used "Random Gatekeeper" today, and I got Hybrid with default suit, even though I own Hybrid Omni suit.

If I remember correctly everyone can select the same gatekeeper at the start there is no first pick / everyone has a unique gatekeeper thing if I'm not wrong so someone getting the same gatekeeper as someone else on random is not a bug
