if the tank wasnt using cooldowns or keeping aggro, then its the tank's fault

if the tank had cooldowns on but still ended up being a wipe, then its the healer's fault

if its neither of these, and you still wipe

then its the DPS's fault

@fading pivot I know this question was a couple of hours ago, but as a controller player I don't actually have a lot of trouble with flipping back and forth between crossbars. You can just enable the WXHB setting if you're really worried. Or you can do what I do and put your main rotational stuff on your main hotbar, then things with longer cds or situational uses on secondary hotbars.
