You'll get sent back to the entrance of the instance, but once the fight is over a shortcut portal will appear that will take you to the boss room for the last boss killed.
Specifically, the portal will appear after the first boss is killed, and the point it takes you to will always be the room of the last boss that was killed.
The exception to this is 24-man raids where there are checkpoints rather than a shortcut which serve a similar function.

Yep, basically only in an "oh shit there's too much damage and the healing isn't keeping up" situation, and even then, you'll want to do something like Arm's Length (20% slow to enemies when they hit you) + Rampart (-20% damage taken) rather than stacking two pure defensives.
