Exact scenario can't be verified, since the achievement triggered for me before Steam Integration, but it appears the (Level 5/10/15 All Operator Shapes) Achievements can trigger early if you reach the specified level for all the Operator Shapes you currently have unlocked, even if there are still more you need to unlock to have "all" of them. This has confirmed to happen in both Manufacture and Classic Game-modes.

T Splitter achievement triggers when 1 splitter is placed, not 25

The T Splitter achievement incorrectly triggers when only 1 Split is placed instead of requiring 25 as intended. This occurs when starting any Savegame that supports achievements and placing a single Split. The achievement unlocks immediately despite its description stating that 25 T Splits must be placed. The expected behavior is for the achievement to only unlock after the player has placed 25 Splits total.

In Classic Mode with the Hard scenario, completing Milestones 8, 9, 10, or 11 incorrectly unlocks Operator Shapes 9, 10, 11, or 12 respectively, causing an off-by-one progression error. The issue can be reproduced by starting a new Classic Mode save in the Hard scenario, opening the Milestones Screen, and using the lock icon to unlock up to Milestone 8. The expected behavior is that each Milestone should unlock the corresponding Operator Shape of the same number.

The Certified achievement is triggered on starting certification

The Certified achievement was reported to trigger immediately upon starting Certification instead of upon completion. The reporter attempted to reset progress by deleting achievements.bin and all savegames, then starting Certification fresh. However, the issue was caused by not also deleting a separate file where the tutorial completion flag is stored. After fully clearing all relevant data, the achievement triggered correctly on Certification completion. This was confirmed as not a bug.
